On the Questionnaires tab, switch the "switch" at the top left to convert the Google form to a quiz. The options are activated as shown on the right of the screenshots.

I advise you to uncheck the display of the correct answers so that learners try to find them on their own and improve their score by modifying their answers. All you have to do is click on the "SAVE" link at the bottom right.

Now, you can move on to the creation of the quiz questions.
